Job Summary

The Flex Care Program Manager provides administrative support to the Flex Care team of Nurse Care Manager and prescribing physicians.  This position assures that the Flex Care protocol is carried out accurately throughout assessment, planning, intervention, and evaluation, state and program reporting requirements.  Ensures that initial screenings of potential patients are completed, assists with assessing insurance coverage, program requests for data, manages Flex Care provider meetings, and collaborates with outside agencies as needed.   

 

Job Functions (may include)

  1. Ensures that program policies and procedures are up to date and correctly implemented, maintains their integrity, monitors patient status assuring safety, and plays a role in the education of clinical staff and patients.

  2. Working with the Nurse Care Manager and Flex Care Medical Director, helps determine whether patients meet program inclusion criteria and assists with patient recruitment from community.

  3. Provides direct patient education regarding program participation and provides additional information to patients as needed to ensure their understanding and adherence.Ensures the integrity of the informed consent process between Flex Care program staff and patient.

  4. Responsible for program coordination activities including screening patients for determination of eligibility, enrollment, scheduling of clinical procedures, and follow up visits many of which require multidisciplinary coordination between other providers as necessary.

  5. Acts as a liaison between primary care and specialty physicians, inpatient/outpatient nurses, and other clinical providers regarding eligibility, enrollment and progress of program subjects.Works collaboratively with other staff.

  6. Participates in conferences related to the Flex Care program.

  7. Initial assessment of social, medical, mental health, substance abuse and medication history.

  8. Follow State and Federal guidelines in providing care to opioid dependent patients in collaboration with licensed, prescribing physicians and pharmacy.

  9. Collects pertinent data using appropriate assessment techniques

  10. Consistently uses clear concise and effective communication written/oral and comply with departmental policies when sharing/documenting relevant patient care data.

  11. Works with the pharmacy and the Flex Care team in obtaining medication orders, prescriptions, refills, and communication as needed

  12. Completes data collection forms for patients and ensures accurate entry into electronic medical record.

  13. Responsible for maintaining the program electronic medical record content.Works with agency IT department on data and user interface correction when needed.Responds in a timely manner to all authorized queries related to the data.

  14. Serves as a liaison and reference to physicians, and other staff members with questions regarding program, procedures and requirements.

  15. Compiles required records and other audit materials as needed.

  16. Ensures that the patient pharmacies are appropriately involved in program.

  17. Conforms to agency standards and regulatory standards (e.g., FDA, DEA, OHRP, etc.) of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.

  18. Utilizes agency values as the basis for decision making and to facilitate the agency’s mission.

  19. Follows established agency policies and procedures, including infection control and safety procedures.

  20. Performs other duties as needed.

 

Qualification Requirements

Education

  1. Position prefers Bachelor Degree in social science or health-related field. Washington State Chemical Dependency Professional or Professional Trainee is preferred.

 

Experience 

  1. 3 + years clinical or administrative experience preferred.   

 

Knowledge Requirements

  1. Computer skills required, preferably with Microsoft Office suite.

  2. Ability at a basic level to input and analyze data and generate reports.

  3. Strong interpersonal skills.

  4. Strong patient communication and patient education skills.

  5. Ability to be highly organized, detail-oriented, and timely.

  6. Ability to work productively across academic clinical departments.

  7. Position requires tact and diplomacy to successfully motivate and educate others.

 

Language Skills

  1. Ability to read and interpret general business correspondence, policies and procedures, referral information, financial documentation and applicable government regulations.

  2. Ability to write business letters, reports, instructions and procedures.

  3. Ability to present information effectively and respond to questions from patients, staff, and stakeholders.

 

Mathematical Skills and Reasoning Ability

  1. Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly.

  2. Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule or diagram format.

  3. Intermediate math skills.

 

Physical Requirements

  1. The employee is regularly required to sit; use hands to finger, handle or feel objects, tools or controls; reach with hands and arms and talk or hear; frequently required to stand, walk and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.

  2. The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
