SPECIALTY CARE COORDINATOR


Description

The Specialty Care Coordinator assists care teams in a multi-disciplinary clinic, serving patients by coordinating all of their external and internal specialty medical visits. This person will work with a team to help under-served patients connect with needed medical appointments, identify ways to overcome barriers, and improve referral processes for their health care. 

This position is 40 hours a week, with full medical and dental benefits and 27 paid days off in the first year. Pay is $15.25/hr. The application deadline is 4/28/17.

Responsibilities

  • Provide referrals support for clinic and outreach medical care teams
  • Prioritize and process referrals, completing all required prior authorizations
  • Verify client insurance and financial assistance programs
  • Coordinate in-house specialty clinics and outside partnerships
  • Locate appropriate external medical offices accepting clients, as needed
  • Share required and authorized medical information and other paperwork
  • Schedule specialty appointments and interpreters as needed
  • Communicate appointment details and instructions to clients
  • Conduct follow-ups for client records, panel managers and medical housing
  • Work with the billing department to navigate the insurance impacts of the referrals process
  • Build and maintain relationships with community social service partners and safety net clinics
  • Participate in referrals program trainings as well as clinic and related meetings
  • Identify and address difficulties and client barriers within referral processes

Qualifications

  • Bilingual English and Spanish fluency preferred
  • Must pass Department of Human Services background check
  • Medical training or experience preferred
  • Strong computer skills including proficiency with Microsoft Word, Excel and Outlook
  • Familiarity with Electronic Medical Record systems such as CPS or EPIC
  • Excellent communication and organizational skills
  • Able to handle multiple tasks, escalated clients and crisis situations
  • Adaptable to changing situations and self-motivated
  • Willing to work with – and advocate for – people from diverse ethnic, cultural, gender, socio-economic  and sexual preference backgrounds
  • Strong reception and administrative skills, demonstrating accuracy and attention to detail
