The Behavioral Health Counselor works as a part of the primary care team to identify and manage patients with behavioral and medical concerns in a primary care setting. The Behavioral Health Counselor uses brief, solution-focused interventions to assess needs, support skill development, and provide psychoeducation. This position acts as a consultant to the primary care provider, providing diagnosis and care plan input and management. The Behavioral Health Counselor will provide some case management as a member of the primary care team. This position is primarily based at a Federally Qualified Health Center (FQHC) in downtown Portland serving adults experiencing homelessness, substance abuse and mental health concerns, and chronic illness. Work may also be performed at outreach sites. 

The position is 20 hours a week. Pay range is $18 (MSW) - $22 (LCSW)/hr. Clinical supervision toward LCSW Licensure provided. This position will be open until filled.


  • Provide behavioral health assessment and brief, solution-focused treatment for clients with medical illness, chronic disease, mental health and/or substance abuse concerns using Cognitive Behavioral Therapy, Motivational Interviewing and/or Dialectical Behavior Therapy. 
  • Coordinate care with other systems and make referrals to services. 
  • Consult with interdisciplinary primary care teams to provide education regarding psychosocial treatments.
  • Ability to provide services at downtown FQHC and at outreach sites (personal vehicle not required, but reimbursement available if used).
  • Provide crisis intervention as needed, including assessment and appropriate referrals. 
  • Maintain accurate and timely documentation for all services provided.


  • MSW; LCSW preferred
  • Experience working with clients and staff from diverse racial, ethnic, cultural, socioeconomic, sexual and gender backgrounds.
  • Knowledge of service systems and clinical issues as they pertain to clients experiencing homelessness, mental illness and/or addictions.
  • Experience conducting mental & behavioral health assessments and treatment.
  • Previous experience in medical health care setting preferred.
  • Knowledge of mental health, addiction and social service resources in the Portland Metropolitan area.
  • Experience and interest in working in an inter-disciplinary team setting. Comfort filling multiple roles and switching roles in a fluid manner.
  • Ability to manage schedule and complete paperwork in a timely manner. Comfort with fast-paced work environment.
  • Proficiency in MS Office Suite and comfort with electronic medical records.
  • Language proficiency in language other than English highly valued.
  • Must be able to work some limited evening hours.
