The REACH mission is collaborating with individuals through outreach, relationships, advocacy and bridging gaps, to reduce harm and support healing. REACH is a program of Evergreen Treatment Services that works with individuals experiencing homelessness and substance use disorders to support clients’ efforts to achieve their goals and improve their quality of life. REACH helps clients’ access essential community resources such as housing, health care, entitlements and behavioral health treatment.

The purpose of this position is to successfully integrate outreach & harm reduction focused substance use disorders services within Public Health’s Mobile Medical Van to assist individuals to achieve improved stability and quality of life.


  • To provide effective prevention and intervention strategies for those most at risk and most in need to prevent or reduce more acute illness, high-risk behaviors, incarceration, and other emergency medical or crisis responses.
  • To increase collaboration between SUD treatment providers, Public Health, and prevention and health service treatment providers.
  • To lower barriers to SUD treatment for homeless individuals and individuals with chronic mental health issues.
  • To provide technical assistance training and consultation to MMV staff regarding SUD issues.

Job Summary

The REACH Harm Reduction & Recovery Specialist will provide on-site substance abuse intervention services, as part of the Mobile Medical Van (MMV) multidisciplinary team, to homeless individuals accessing MMV Services. Services provided from a harm reduction and motivational interviewing perspective will include outreach, engagement, SUD assesment and linkage to SUD treatment and other recovery support, care coordination and support to help clients address problematic substance use behavior and access needed services.

The CD/CDPT Specialist must develop and maintain productive relationships with the MMV team. The nature of this position is extremely independent, requiring creative thinking, multitasking, excellent interpersonal relationship skills and conflict resolution skills.

Job Functions (May Include)

  1. Provide substance abuse intervention services for individuals accessing MMV services.
    • Provide outreach to potential clients to explain offered services and establish a working relationship.
    • Develop rapport with potential clients through establishing a friendly presence at MMV sites and responding to questions and concerns presented by individuals.
    • Assess nature and severity of clients’ substance use/dependency and match clinical interventions to each individual’s stage of change.
    • Develop and implement, with the client’s input, an individualized Service Plan which addresses the client’s stated needs and priorities and focuses on reduction/elimination of harm associated with drug/alcohol use through self-change methods.
    • Conduct SUD assessments and link individuals to inpatient and outpatient SUD treatment.
    • Develop and maintain a productive working relationship with MMV collaborators including meal program staff and with community resources in order to provide effective coordination of client care.
    • Provide services consistent with ETS/REACH program policies and procedures.
  2. Advocate for the client with other service providers:
    • Assist clients with accessing needed services, including medical detox and inpatient SUD treatment when indicated.
  3. Identify gaps and barriers in available community resources and advocate for systemic changes.
  4. Attend REACH and MMV team meetings and other required meetings.
  5. Develop and maintain client files according to contract and state requirements.
  6. Collect data and report on activities as required by contract.
  7. Local travel on behalf of the Agency is a job requirement.
  8. Additional duties as assigned.

Qualification Requirements


Academic training in the social service field and in the area of Substance Use Disorders as required in accordance with the qualifications for a Department of Health Licensed Chemical Dependency Professional (C.D.P.) as stipulated in the WAC. C.D.P.T. status with relevant experience may be considered. Experience working with a chemically dependent population may be substituted for some or all the degree requirements.


A minimum of two years’ experience in work related to social work or outreach programs. Experience providing services to addicted individuals from a harm reduction perspective is essential.

Knowledge Requirements

  1. Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.

  2. Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.

  3. Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.

  4. Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.

  5. Must be able to pass a Washington State Patrol criminal background check.

  6. Flexible team player.

  7. Excellent attention to detail.

  8. Ability to work very independently and seek supervision when appropriate.

Language Skills

  1. Ability to read and interpret general business correspondence, policies and procedures, referral information, financial documentation and applicable government regulations.

  2. Ability to write business letters, uncomplicated reports, instructions and procedures.

  3. Ability to present information effectively and respond to questions from clients, staff, referral sources and the general public.

Mathematical Skills and Reasoning Ability

  1. Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly.

  2. Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule or diagram format.

  3. Basic math skills

Physical Requirements

  1. The employee is regularly required to sit; use hands to finger, handle or feel objects, tools or controls; reach with hands and arms and talk or hear; frequently required to stand, walk and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.
  2. The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.

  3. Valid Driver’s License and acceptable driving record required.
