Job Summary:

The REACH Harm Reduction & Recovery Specialist will provide on-site substance abuse intervention services, as part of the Mobile Medical Van (MMV) multidisciplinary team, to homeless individuals accessing MMV Services. Services provided from a harm reduction and motivational interviewing perspective will include outreach, engagement, SUD assesment and linkage to SUD treatment and other recovery support, care coordination and support to help clients address problematic substance use behavior and access needed services.

The CD/CDPT Specialist must develop and maintain productive relationships with the MMV team. The nature of this position is extremely independent, requiring creative thinking, multitasking, excellent interpersonal relationship skills and conflict resolution skills.

Job Functions Include:

  • Provide substance abuse intervention services for individuals accessing MMV services
  • Provide outreach to potential clients to explain offered services and establish a working relationship
  • Assess nature and severity of clients’ substance use/dependency and match clinical interventions to each individual’s stage of change
  • Develop and implement, with the client’s input, an individualized service plan which addresses the client’s stated needs and priorities and focuses on reduction/elimination of harm associated with drug/alcohol use through self-change methods
  • Conduct SUD assessments and link individuals to inpatient and outpatient SUD treatment
  • Advocate for the client with other service providers

 

Required :

  • Experience working with people who have substance disorders is required
  • Harm reduction approach 
  • Two years’ experience in work related to social work or outreach programs

 

Preferred:

  • Licensed Chemical Dependency Professional (C.D.P.) 
  • C.D.P.T. status with relevant experience 
  • Experience providing services to addicted individuals from a harm reduction perspective
