Purpose

The purpose of the Outreach Care Coordinator position is to engage and support individuals experiencing homelessness and behavioral health challenges to access services they need to stabilize and make progress toward housing and increased quality of life.

Job Summary

This position involves street outreach to make contact with individuals who are living outside, in order to build rapport, address immediate needs, and provide assistance with accessing long term services. This position will collaborate with REACH staff, local Veteran’s services and other community resources, first responders, and other outreach providers, and will engage eligible individuals who may be referred or identified through street outreach. The REACH worker will meet with clients on the street or at other locations and may accompany clients to community resources as needed to ensure engagement in services.

 

The target population is adults who have served in the military and are experiencing homelessness and behavioral health challenges who are not engaged in services needed to improve their quality of life. The Outreach Care Coordinator will work with individuals who face multiple barriers and are traditionally underserved to access the local behavioral health and primary care services they need. Services provided will be client-centered and strength-based, utilizing motivational interviewing skills and delivered from a harm-reduction and trauma informed perspective, focused on client choice and improved quality of life.

The Outreach Care Coordinator must develop positive and collaborative relationships with partners and staff of other agencies providing services to this population in order to effectively coordinate care. The nature of this position is extremely independent, requiring initiative and creative thinking, multitasking, strong time management and organizational skills, and conflict resolution skills. Excellent boundaries and ability to conduct street outreach while maintaining personal safety through best practice and awareness is essential.       

Job Functions (May Include)

  1. Provide outreach, engagement and care coordination for eligible individuals.
    • Provide outreach to potential clients to explain offered services and establish a working relationship.
    • In collaboration with the client, develop an individualized plan which addresses the client’s stated needs and goals.
  2. Meet clients where they are, on the streets, in their living situation, or at other service facilities, to explain offered services and establish a working relationship.
  3. Develop rapport and a trusting relationship with clients through respectful outreach and client-centered engagement practices.
  4. Assist clients in reducing barriers and increasing access to housing. Supporting clients in navigating housing systems such as CEA and the VA HUD/VASH program.
  5. Help facilitate clients’ access to primary medical care at the VA or at other clinics.
  6. As appropriate, address clients’ substance use issues and match clinical interventions to each individual’s stage of change, supporting their goals and focusing on reducing the harmful consequences of substance use through self-change methods.
  7. Address clients’ needs for behavioral health treatment services and facilitate engagement in appropriate supportive services including the VA when applicable.  
  8. Support clients’ efforts to move toward financial stability through access to entitlement programs, protective payeeships, educational and vocational programs, and employment opportunities.
  9. Develop and maintain productive working relationships with community programs which may support clients to provide effective coordination of care.
  10. Provide services consistent with ETS/REACH program policies and procedures.
  11. Assist clients in keeping appointments.
  12. Advocate for the client with a wide variety of other service providers:
    • Assist with accessing needed services by setting up and attending appointments with clients if needed to provide continuity of care.
  13. Identify gaps and barriers in available community resources and advocate for systemic changes.
  14. Attend REACH meetings, and other required meetings.
  15. Utilize clinical supervision, psychiatric consultation and peer support for exploring clinical intervention strategies, counter-transference, resource development, self-care and burnout prevention.
  16. Document all services provided according to agency, contract and state requirements.
  17. Local travel on behalf of the agency is a job requirement.
  18. Additional duties as assigned.

Qualification Requirements
 

Education

High school diploma or equivalent required.  Undergraduate degree preferred. Relevant experience may substitute for degree. Academic training in the social service field and in the area of behavioral health/substance use disorders desired. CDP/CDPT and MSW’s highly desirable.

 

Experience

A minimum of three years of experience in work related to social work or outreach programs. Direct experience providing services from a harm reduction perspective to individuals experiencing behavioral health challenges essential. Knowledge and understanding of the Veteran’s Administration system and benefits; ability to support clients in navigating this system to access services.

 

Knowledge Requirements

  1. Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.
  2. Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.
  3. Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.
  4. Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.
  5. Must be able to pass a Washington State Patrol criminal background check.
  6. Flexible team player.
  7. Excellent attention to detail.
  8. Ability to work very independently and seek supervision when appropriate.

 

Language Skills

  1. Ability to read and interpret general business correspondence, policies and procedures, referral information, financial documentation and applicable government regulations.
  2. Ability to write business letters, uncomplicated reports, instructions and procedures.
  3. Ability to present information effectively and respond to questions from clients, staff, referral sources and the general public.

 

Mathematical Skills and Reasoning Ability

  1. Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly.
  2. Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule or diagram format.
  3. Basic math skills.
     

Physical Requirements

  1. The employee is regularly required to sit; use hands to finger, handle or feel objects, tools or controls; reach with hands and arms and talk or hear; frequently required to stand, walk and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.
  2. The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.

Valid Driver’s License and acceptable driving record required.
