Job Summary:

This position involves street outreach to make contact with individuals who are living outside, in order to build rapport, address immediate needs, and provide assistance with accessing long term services. This position will collaborate with REACH staff, local Veteran’s services and other community resources, first responders, and other outreach providers, and will engage eligible individuals who may be referred or identified through street outreach. The REACH worker will meet with clients on the street or at other locations and may accompany clients to community resources as needed to ensure engagement in services.

Job Functions Include:

  • Provide outreach, engagement and care coordination for eligible individuals.
  • Provide outreach to potential clients to explain offered services and establish a working relationship.
  • In collaboration with the client, develop an individualized plan which addresses the client’s stated needs and goals.
  • Meet clients where they are, on the streets, in their living situation, or at other service facilities, to explain offered services and establish a working relationship.
  • Develop rapport and a trusting relationship with clients through respectful outreach and client-centered engagement practices.
  • Help facilitate clients’ access to primary medical care at the VA or at other clinics.


  • High school diploma or equivalent, Relevant experience may substitute for degree
  • Three (3) years of experience in work related to social work or outreach programs Knowledge and understanding of the Veteran’s Administration system and benefits
  • Valid Driver’s License and acceptable driving record


  • Undergraduate degree
  • Academic training in the social service field and in the area of behavioral health/substance use disorders
  • MSW
