About REACH’s Outreach Team

REACH Outreach Care Coordinators conduct assessments and provide referrals to services such as case management, housing placement, mental health care, and medical care. REACH services are person-centered, strengths-based, trauma-informed, culturally relevant, and respectfully provided from a harm reduction perspective.


Outreach Care Coordinator Job Summary

The purpose of this outreach is to offer information, referral and direct supportive services to help these individuals access needed community resources. Though the outreach may be conducted with a team member, the nature of this position is extremely independent, requiring excellent interpersonal relationship and communication skills, common sense regarding personal safety, creative thinking, multitasking, and conflict resolution skills. This position entails going out into the community to contact, develop rapport, and engage with individuals who are living outside. Outreach Care Coordinators are designated to particular neighborhood areas in order to maximize opportunities to connect with individuals living outside.


Outreach Care Coordinator Job Functions:

  • Going to public parks, greenbelts and streets in order to contact individuals living outside and explain offered services

  • Developing relationships with individuals in order to assess their service needs and respond with appropriate referrals and linkages to resources

  • Helping individuals access community resources, such as shelter, housing, entitlements, medical care, etc.

  • Referring eligible individuals to more intensive case management programs when indicated

  • Develop effective relationships with shelters and other programs that provide direct services to homeless persons

  • Identify gaps and barriers in available community resources and advocate for systemic changes to improve quality and coordination of community services



  • Two years of experience in work related to social work or outreach programs

  • Ability to sit and stand for long periods of time, occasionally lift up to 30 lbs., and work outside in all weather conditions

  • High school diploma or equivalent

  • Valid Driver’s License and acceptable driving record



  • Undergraduate degree with coursework in a social service or related field

  • Experience with harm reduction approaches and motivational interviewing
