About the Position

The function of this position is to provide outreach, engagement and intensive case management/care coordination to Spanish-speaking individuals who are experiencing substance use disorders, homelessness, and face barriers to accessing needed resources. The purpose of these services is to help individuals achieve psychosocial stability and improved quality of life. Client-centered services are provided from a harm reduction perspective utilizing motivational interviewing skills.

 

Bilingual Housing Case Manager Job Summary:

  • Meet clients where they are, either on the streets or at other service facilities (including jail), to explain offered services and establish a working relationship.

  • Provide outreach and housing-focused case management services in English and Spanish in a culture-informed and trauma-informed manner, with particular sensitivity to issues regarding immigrants and refugees.

  • Assist clients in obtaining housing and maintaining housing stability.

  • Assess client’s needs for mental health, primary medical care, and substance use according to their stage of change, focusing on reducing harmful consequences of behavior.

  • Advocate for clients with a variety of other service providers, including but not limited to Coordinated Entry for All (CEA), other housing resource and/or voucher programs, and clients’ landlords.

  • Provide ongoing support to clients once they are housed to maintain housing and improve quality of life.

 

Must-have’s:

  • Bilingual with a working fluency in both English and Spanish

  • Experience working with non-English speaking immigrants and/or refugees in a social services setting.

  • Minimum of two years’ experience in case management related to people experiencing homelessness and co-occurring behavioral health conditions.

  • Experience providing services to individuals with substance use disorders from a harm reduction perspective.

 

Nice-to-have’s:

  • Experience working with undocumented individuals as they navigate the immigration system.

  • Undergraduate degree in a field related to social services – can be substituted by years of relevant case management experience.

Salary: $52,000.00 /year
