About the Position
The purpose of the Outreach Care Coordinator position is to engage and support individuals experiencing homelessness and behavioral health challenges to access services they need to stabilize and make progress toward housing and increased quality of life.
The target population is adults who have served in the military and are experiencing homelessness and/or behavioral health challenges. Services provided will be client-centered and strength-based, utilizing motivational interviewing skills, and delivered from a harm reduction and trauma-informed perspective. We focus on the client’s choice and improved quality of life.
Outreach Care Coordinator – Veteran’s Specialist Job Summary:
Meet clients where they are, either on the streets or at other service facilities (including jail), to explain offered services and establish a working relationship.
Provide outreach and case management to eligible individuals by explaining offered services and developing an individualized plan that addresses the client’s stated needs and goals.
Assess client’s needs for mental health, primary medical care, and substance use according to their stage of change, focusing on reducing harmful consequences of behavior.
Advocate for clients with a variety of other service providers, including but not limited to Coordinated Entry for All (CEA), the Veteran’s Administration (VA), HUD-VASH, or other housing resources and/or voucher programs.
Minimum of three (3) years of experience in social work or outreach programs
Knowledge and understanding of the Veteran’s Administration system and benefits, with the ability to support clients navigating this system
Direct experience providing services from a harm reduction perspective to individuals experiencing behavioral health challenges
High school diploma or equivalent
Valid Driver’s License and acceptable driving record
Undergraduate degree related to social services or equivalent years of relevant education
Academic training in behavioral health and/or substance use disorders (e.g. MSW, CDP/CDPT)
