  • Our Client is a well-established Healthcare Organization.


  • Provision of casework management and counselling through social work assessments and intervention in the management of ESRD patients (e.g. BPSS, Risk & Needs Matrix, Case Intervention Plans, etc.) in the context of both medical knowledge in community dialysis setting.
  • Delivery of social work services and programmes to ESRD patients in need and their families.

Key Responsibilities:

  • Manage ESRD patients in Dialysis Centre (DC) r Home Dialysis (HD) through client engagement, conducting interviews, data collection, case assessment, case conceptualization, case planning and intervention plans for best patients outcomes.
  • Identify and presenting underlying issues/problems to design a care plan to enhance/maintain and promote psychosocial functioning of patients.
  • Conduct FFRS (Financial Fee Reviews) and placement on Company's subsidies and welfare programmes.
  • Conduct visit to Dialysis Centres, home or hospitals to provide supportive counselling.
  • Advocate for the protection of patients' dignity, confidentially and rights.
  • Collaborate with Healthcare professionals internally and externally in total patients management.
  • Network with Government, community, social agencies and/or individuals to mobilize social resources to meet patients's needs.
  • Facilitate patient and caregiver support programmes.
  • Document bio-data of patients and interventions in clinical system for records, complete timely online case documentations (e.g. case session recordings, follow-up recordings, risk and needs assessment, social reports, patients' feedback form, etc.).


  • Diploma or Degree in Social Work.
  • Must be a Registered Social Worker (RSW) with the Singapore Association of Social Workers.
  • At least 3 to 4 years' direct experience in Social Work practice, from Social Service Agency.
  • Patients-oriented.
  • Excellent communication and interpersonal skills.
  • Possess good direct Case Management experience.
  • A team player.
  • Willing to handle 2 Dialysis Centres.

Other Information:

  • Working Location: Toa Payoh/Balestier.
  • Working Hours: Mondays to Thursday, 8.30am to 6pm and Fridays, 8.30am to 5pm.
  • Critical: Due to the organization's affirmed support and commitment to developing a Singaporean core, this role is only available to Singaporeans.
