Our Client is a well-established Healthcare Organization.
Provision of casework management and counselling through social work assessments and intervention in the management of ESRD patients (e.g. BPSS, Risk & Needs Matrix, Case Intervention Plans, etc.) in the context of both medical knowledge in community dialysis setting.
Delivery of social work services and programmes to ESRD patients in need and their families.
Manage ESRD patients in Dialysis Centre (DC) r Home Dialysis (HD) through client engagement, conducting interviews, data collection, case assessment, case conceptualization, case planning and intervention plans for best patients outcomes.
Identify and presenting underlying issues/problems to design a care plan to enhance/maintain and promote psychosocial functioning of patients.
Conduct FFRS (Financial Fee Reviews) and placement on Company's subsidies and welfare programmes.
Conduct visit to Dialysis Centres, home or hospitals to provide supportive counselling.
Advocate for the protection of patients' dignity, confidentially and rights.
Collaborate with Healthcare professionals internally and externally in total patients management.
Network with Government, community, social agencies and/or individuals to mobilize social resources to meet patients's needs.
Facilitate patient and caregiver support programmes.
Document bio-data of patients and interventions in clinical system for records, complete timely online case documentations (e.g. case session recordings, follow-up recordings, risk and needs assessment, social reports, patients' feedback form, etc.).
Diploma or Degree in Social Work.
Must be a Registered Social Worker (RSW) with the Singapore Association of Social Workers.
At least 3 to 4 years' direct experience in Social Work practice, from Social Service Agency.
Excellent communication and interpersonal skills.
Possess good direct Case Management experience.
A team player.
Willing to handle 2 Dialysis Centres.
Working Location: Toa Payoh/Balestier.
Working Hours: Mondays to Thursday, 8.30am to 6pm and Fridays, 8.30am to 5pm.
